Probe Sheet 1: This is a common probe data sheet. Probe data can be taken in a number of ways. This probe sheet can be used to record the first trial of the day as either correct (+) or opportunity to learn (-) across three different stimulus conditions. These stimulus conditions can be different teachers, different objects etc.

Probe Sheet 2: This is a common probe data sheet used for daily probe data collection.

Teaching Objective Sheet: This sheet can be used in a program to describe the teaching of a specific skill. It can also be used to transfer the teaching skill to other teachers/therapists. It includes most of what should be thought of during teaching including: skill or objective; criterion for mastery; type of measurement; specific direction or discriminative stimulus; prompting to be used; and consequences for a variety of behaviors that may occur including correct responses. The second page of this document can be used to describe changes to the original teaching plan.

Mand Data Page: This page is designed for vocal mands. Trails are numbered along the bottom row. A plus (+) can be placed in the box for each trial that corresponds to the appropriate controlling variable(s).

Functional Assessment and Behavior Treatment Plan: This document was originally developed for a school district in Florida in 1997 to satisfy the mandates that resulted from the reauthorization of I.D.E.A. The reauthorization stated that students who exhibit aberrant behavior still have the right to FAPE (Free and Appropriate Public Education). It mandated that if behaviors interfere with learning, a functional assessment and behavior treatment plan need to be developed. This document outlines a five step process that leads to a behavior plan to reduce inappropriate behaviors and teach alternative behaviors.
